Alcalá de Henares Launches Pioneering System Connecting Hospital and Primary Care

The initiative aims to improve care coordination and reduce diagnosis times in the area served by Príncipe de Asturias Hospital.

The Community of Madrid has implemented an innovative system in Alcalá de Henares, linking the Príncipe de Asturias Hospital with Primary Care, aiming for real-time care continuity and improved communication among professionals.

This new model, named AProxima, is designed as a meeting point and active listening space for professionals at both levels of care. It involves the participation of health centers, consulting rooms, continuous care points (PAC), and socio-health services within the reference area.
The main objective is to optimize communication channels to facilitate a more agile exchange of information among healthcare staff. Additionally, it envisions the creation of more efficient circuits to shorten diagnosis times and reduce unnecessary visits to hospitals and other health centers.
Since its launch in November 2025, five new medical e-consultations have been generated in specialties such as Ophthalmology, Traumatology, Clinical Analysis, Soft Tissue Tumor Surgery, and Vascular Surgery. These are complemented by a monographic Nursing consultation for ostomy management, as well as several high-resolution consultations in areas like Rheumatology and Ophthalmology.
In parallel, new communication channels have been established among professionals, including periodic newsletters and the creation of a common web space on the intranet of the Hospital Universitario Príncipe de Asturias. This portal already hosts numerous protocols, documents, and audiovisual material on pathologies across more than 30 specialties, fostering collaboration and improving care practices.
The project also includes training activities to promote cooperation among teams. In this context, the Innovation Day event will be held on May 27, where initiatives aimed at enhancing care coordination will be recognized.
This system builds upon a pilot project developed in 2022 by the hospital's Microbiology Service, which focused on the early detection and treatment of sexually transmitted infections through rapid diagnostic techniques and more direct communication with Primary Care.
